Getting a Dosimeter
Step-by-Step Process
- Go to the Dosimeter Request Form (DRF)
Available online (https://www-bis2.slac.stanford.edu/drf/). The link can also be found on ESH Forms. - Log in using your credentials
SLAC login, federated ID, or SLAC guest account. - Complete the request form
The system will automatically select the correct form (Form A, B, or V) based on your SLAC training status. - Meet training requirements
Course 115 (GERT) is required for most standard dosimeter requests. - Submit for approval
Your Point of Contact (approver) or Radiation Protection (RP) will review the form. Pick up your dosimeter
Go to the Badging Office once approved. The Badging Office is in the VUE Center in Building 53.Your name will be on the dosimeter issuance list—no extra paperwork needed.
Special Cases
- Tour dosimeters: Requires a special paper form C.The link can also be found on ESH Forms.
Tour coordinators should fill this out and submit to esh-DREP@slac.stanford.edu. - Escorted users without required training: An escort may request the dosimeter for a person who does not have the training. The escort must stay with their person while they are on site in Controlled Areas.
- Unescorted access to Radiological Controlled Areas (RCAs): Unescorted access requires a SLAC ID badge, SLAC online account, training (Course 115), and submittal of the Dosimeter Request Form.

Forms and Requirements
- Form A is for everyone who has taken Course 115 and has the appropriate SLAC computer credentials.
- Form B is for people who do non-radiological work in a RCA and do not have training.
- Form C is for Tours at SLAC. A Form C dosimeter is requested by the Tour Guide, who will accompany the tour group at all times in SSAs at SLAC, and the dosimeter is returned to Radiation Protection promptly at the conclusion of the tour.
- Form V is for individuals without a SLAC ID number, who need to be escorted into RCAs. The dosimeter will be assigned to the escort, and worn by the escorted individual.
Note: The system will automatically select the correct form (A, B, V) based on your training. (Form C is a special paper request.)
Course 115 (GERT) is the minimum required training for most dosimeters at SLAC.
Radiological work or access to radiological areas requires Course 116.
Exceptions for training are made for tours and escorted individuals (see form types, above).
Accounts and Access

- A SLAC account is required, whether it is a regular SLAC computer account, or a limited SLAC guest account.
- Regular hires or scientific users should go through their department’s procedures for a regular SLAC computer account.
- For a guest account, the point of contact at SLAC must invite their guest through the SLAC Guest Gateway [link to be live 6/26]. Once the account is created, see the steps above for obtaining a dosimeter.
- Each person must submit their own request for unescorted access to an RCA (Form A)
- A person with GERT training can submit a request for an escorted person without GERT training (Form B: escorted, no radiological work), or for a person without a SLAC ID (Form V)
- A paper form C can be submitted for a tour group.
Submitting and Managing Requests
- Submit a new form, or

- No, only administrators can delete it

- Use the email link provided after the form was submitted
Pickup and Use
- Badging Office
The Badging Office is in the VUE Center in building 53. Security Office hours are 7am to 5pm, Monday through Friday (closed on some holidays). The VUE Center is on the first floor, on the left side as you come in the building.
- Submit a new request for a dosimeter for yourself – the system will ask you if this is a replacement, and the Radiation Protection department will follow up for details.

- Yes; this system is accessible outside the VPN. It only requires a SLAC Login (or Stanford, or Federated Institution, or SLAC Guest)
